Because I like this idea and I think about this every so often. Here's my take on what we could do.
We have here a map of Central Indiana.
Northern Steppes
Kanthea
Stormhaven
Mittelmarch
Implementation:
--Every two weeks we head out to attack an adjacent hex or defend an owned hex from an attacker.
--We set up a schedule (viewable on all necessary forums) for our practices to jive with another realm's so we can have our domination practices.
--With the use of the forums, we set up our battles. Each respective realm authority (council, monarch, etc.) will negotiate the play styles and victory/loss conditions. For example if Mittelmarch comes up to Stormhaven, Ematai, Spoons, and Elik negotiate with Nichtmar, Rune, Baethor, and Tari that they will set up 5 battles 2 of them or King Battles, 1 is a CtF with Bridge battle, and 2 more would be Stand and Deliver.
--In each tile there should be an environment setting attached to it. If the tile is listed a forest, then we fight a woods battle with the respective games or a water tile would mean boat battles.
--Loss of a capital tile would have negative ramifications to the defender. If Stormhaven loses its capital, then we could no longer initiate attacks on realms outside our original realm and we would have to try to regain the capital in order to do so. The loss of a capital tile could even mean we would have to make an effort to assist our ruling realm in a battle against another realm. For example if Mittelmarch conquers 75% of Stormaven's hexes and Mittelmarch then decides to attack an adjacent Northern Steppes tile, we would then be encouraged to join them. Problems: How to take back a capital tile while in possession of zero original tiles? Perhaps like was Sabo said with uprisings. Keeping up with who owns what and logistics for caravan-ing to said destination to assist with our ruling realm.
--Other realms like Dunland, Aethenu, Bloodpool, whoever may be adjacent to another realm listed in my map may add their realm to the next logical spot, ideally they'll be positioned to 2 separate realms so that they can fight two different sets of people.
--Thoughts on making the teams fair? We can't just "bench" fighters to make the fighting fair. Should it be left as is so that if we bring an army of 20 fighters to Kanthea and they bring 10, do we say 'all is fair in love and war'? I say why not?

Aside from that, we drop most of what Darkon has and keep with Dagorhir rules. Although I am very impressed with that Darkon documentary.
